I give my permission for the VBS staff to administer basic first aid to my child in the event of injury. If my child needs immediate medical help beyond basic first aid, our procedure is to take the child to Royal Inland Hospital. I therefore give my consent for my child to be taken to Royal Inland Hospital, if necessary, by the VBS Staff if I cannot be contacted. I consent to an ambulance being called to transport my child, if necessary, I further agree to pay all costs incurred for transport.

I hereby grant First Baptist Church permission to take and use photographs/videos taken at VBS of my child for any purpose lawful at the time. It is the practice of First Baptist Church to take photographs of participants the first day to keep on file for safety purposes. It is the practice of First Baptist Church to not publish any photographs of minors to any forms of social media.
